1)Do you have any of the following symptoms (fever, chills, repeated shaking with chills, cough, difficulty breathing, shortness of breath, sore throat, muscle pain, headache, recent loss of smell or taste, vomiting or diarrhea)?
2)Have you been in contact with someone who had symptoms of COVID-19 but was not tested.
3)Have you been in contact with someone who has tested positive for COVID-19?
4)Have you been diagnosed with one of the following (lung or heart disease, diabetes, any immune disease)?
When you arrive, please wait in the parking lot or in our downstairs lobby. Text your license plate number and your name to the number posted on our signage (303-963-6163) and wait to be called back to complete a health screening over the phone. Your temperature will be taken when you arrive on our floor.
